1. Patient Information
Patient: Female, 67 years old.
Preoperative Diagnosis: Papillary carcinoma of the left thyroid lobe; Nature of right thyroid mass to be determined.
Procedure Performed: Left radical thyroidectomy for carcinoma + Right thyroid lobectomy + Intraoperative recurrent laryngeal nerve monitoring.
2. Monitoring Purpose
The application of Intraoperative Neurophysiological Monitoring (IONM) allows for real-time functional assessment of the Recurrent Laryngeal Nerve (RLN) and Vagus Nerve (VN). This assists the surgeon in accurately identifying and effectively protecting these nerves, significantly reducing the risk of postoperative vocal cord paralysis and swallowing dysfunction, thereby enhancing surgical safety and efficacy.

4. Monitoring Items and Methodology

Methodology: Continuous dynamic monitoring of Vagus and Recurrent Laryngeal Nerve function via intraoperative electrical stimulation and electromyographic signal acquisition.
5. Surgical and Monitoring Results
The surgical procedure was completed successfully. Monitoring signals were clear and stable throughout the operation, with no abnormal discharges or waveform loss observed.
Both recurrent laryngeal and vagus nerve functions remained intact. The patient's postoperative recovery was uneventful, with a clear voice and no respiratory or swallowing difficulties.
